Accessing quality healthcare shouldn't require waiting rooms or long commutes. More patients, especially older adults and those in assisted living communities, are turning to housecall medicine for its convenience, comfort, and continuity of care. With in-home primary care, providers bring medical services directly to where patients live, ensuring their needs are met in a more personal, attentive setting.
If you’re considering transitioning to a housecall model of care, here’s what a first visit typically looks like and how GreenWell Health makes the process easy and patient-centered.
A Personalized In-Home Approach
Your first visit starts right at your residence, whether that’s your home, an assisted living facility, or a group home. Instead of navigating traffic or crowded clinics, you’ll receive care in the comfort and familiarity of your own space.
Appointments are scheduled on a consistent, cyclical basis. Our providers give a block of time (either 8 AM to 12 PM or 1 PM to 5 PM) for when they’ll arrive, much like a cable service appointment. You’ll also receive an appointment card at each visit with the date and time of your next one.
What Happens During the First Visit
Your first house call primary care appointment is designed to help us understand your health needs and establish a relationship with your primary care provider. Here's a breakdown of what typically takes place:
Health History Review: Your provider will sit down with you (and caregivers or family if applicable) to go over your medical history, medications, past procedures, allergies, and health concerns.
Physical Assessment: A basic physical exam is performed to assess your current health status. This helps identify any underlying issues or areas that need closer monitoring.
Care Plan Discussion: Based on your health goals and concerns, your provider will begin forming a care plan tailored specifically to you. This may include lifestyle recommendations, follow-ups, or specialist referrals.
Coordination with Care Team: If you're in a community or have caregivers, your provider will communicate with them to ensure everyone is aligned in your ongoing care.
Q&A Time: You’re encouraged to ask questions about anything—from medications and symptoms to preventive screenings and wellness strategies.
Routine Services You Can Expect
GreenWell Health’s primary care program provides all the core services you’d expect from a traditional clinic—delivered directly to you. These include:
Preventive screenings
Management of chronic illnesses
Immunizations
Medication refills and reviews
Lab orders and diagnostic referrals
Coordination with specialists and therapists
Each visit builds upon the last to keep your health on track and ensure continuity of care without disruption.
Advanced and Ongoing Care Options
In addition to routine care, we offer specialized programs tailored to those who need long-term or complex support:
Chronic Care Management (CCM): We support patients with multiple chronic conditions through a coordinated care strategy that includes regular check-ins, medication management, and collaboration with specialists.
Remote Patient Monitoring (RPM): Devices placed in your home help us monitor vital signs like blood pressure, blood sugar, or weight daily. This allows for early detection and real-time adjustments to your care plan.
Gero-Psych and Pain Management: Our providers are trained in geriatric psychiatry and pain management, supporting both mental and physical health for aging adults.
Transitional & Wound Care Management: Whether you’re recovering from a hospital stay or managing wounds, we guide your healing journey with ongoing attention and care.
Additional At-Home Services
We go beyond traditional primary care to make your healthcare as seamless as possible:
Mobile Laboratory: Need bloodwork? No need to leave your home. A phlebotomist or medical assistant will come to you to collect samples for lab tests your provider orders. Results are shared and reviewed during your next appointment.
Mobile Imaging: Through partnerships with local imaging providers, we can order and schedule in-home X-rays, ultrasounds, EKGs, and other diagnostic scans. This eliminates the need to travel to an imaging center.
Pharmacy Services: We work with a mobile pharmacy that prepares and delivers medications directly to your residence. In 2024, we plan to launch our own in-house pharmacy to simplify access even further.
Podiatry (select locations): Routine foot care is essential, especially for patients with diabetes or mobility challenges. We coordinate in-home podiatry visits to keep you moving safely.
Dentistry (select communities): In certain assisted living settings, we partner with local dental providers who make house calls for routine exams and oral care needs.
